Dr. Plotsky is a provider for
the following medical insurance plans. We do not participate in vision care plans.
Since we are committed to providing you with the best possible service, we must
emphasize that as your ophthalmology provider our relationship is with you, and
not your insurance company. We encourage you to inquire and explore your benefits
with your employer or insurance carrier. Not all services are covered benefits in
all contracts. These services are due and payable at the time services are rendered. All copayments, co-insurance, and deductibles will be collected at the time of your visit.
Examples of non-covered services are refractions, contact lens evaluations, and
normal eye exams. The term “routine vision exam” is a term created by insurance
companies and we do not perform these exams. Most PPO’s do not cover normal exams.
These services are due and payable at the time services are rendered.
If your plan requires a referral to a specialist you must bring the referral
with you to your appointment. Services cannot be provided without one. We will accept

As a courtesy to our patients, we file claims to the following insurance companies
with whom we are participating providers:
